You may print and mail your completed application or if time is important, copy items 1-16 below and paste in the body of an email, fill in the information in your email, and send to our Exam Director Jonni Cooper at: jcooper17@tampabay.rr.com
If you do not wish to
submit your payment electronically, please send a personal check or money order
with your mailed application. Keep in mind this will delay your
application and we have a short lead time for the Houston site.
Due to this short lead time on this exam site, we recommend everyone pay online
and fill out the application online. You will however have to bring a copy
of your driver's license to the exam site to place in our paper file.
Bobbi Bryant will be onsite to collect these.

As soon as the ABCM receives your
application and payment, they will email
your confirmation.
You must provide the onsite exam proctor a copy of your valid driver's license or other State photo ID in
order
to be admitted to the exam.
Refund
Policy: There are no refunds once the exam has been entered
or taken. The Board will consider all extenuating circumstances for
refunds, but
there are absolutely no refunds available once you have entered the online exam,
even if you do not complete the exam. So be sure to allow enough time for
your
exam and do not bring your cell phone to the exam room.
To pay online, select the level of the Exam you wish to
take and click on the add to
cart button. Since there is a considerable amount of time to process a
site as large as Houston, the results will not be
available for 3-4 weeks after your exam date.
