Certification Exam Application

You may print and mail your completed application or if time is important, copy items 1-17 below and paste in the body of an email, fill in the information in your email, and send to our Exam Director Jonni Cooper at: jcooper17@tampabay.rr.com

If you do not wish to submit your payment electronically, please send a personal check or money order with your mailed application.

As soon as we receive your application, by email or regular mail, and your payment, you will be emailed a username and password, and the location in your city to sit for your board exam.

Exam applications will be accepted Monday through Friday. The online exam offices are closed weekends, and major holidays.

For emailed applications, this process should not take more than 3 business days to completely arrange.

For mailed applications, this process should not take more than 15 business days to arrange.

The CVRN Level I and II Exams are NOW AVAILABLE ONLINE.

As soon as the ABCM receives your application and payment, they will email
you a username and password, the location to report to take the exam, and your
exam proctor's name and phone number to arrange your exam.

You must show the proctor your valid driver's license or other State photo ID in order
to be admitted to the exam.

Refund Policy: There are no refunds once the exam has been entered
or taken. The Board will consider all extenuating circumstances for refunds, but
there are absolutely no refunds available once you have entered the online exam,
even if you do not complete the exam. So be sure to allow enough time for your
exam and do not bring your cell phone to the exam room.

To pay online, select the level of the Exam you wish to take and click on the add to
cart button (be sure to pay attention to the online vs. pen and paper exams as the price is different for each). The results of your online exam is available as soon as you complete the exam. Since there is a considerable amount of work goes into a pen and paper exams and the results will not be available for 4 weeks after your exam date.

CVRN Level I Board Certification Examination
(Telemetry, PCU, PACU, Step-Down Units, Chest Pain Center, etc.)

To take your exam online, click on the add to cart button; associated cost is $220.00. Allow up to 3 hours for this
examination; the average exam time has been 2 hours. The exam is timed and will automatically end without notice.

To take your exam by pen and paper, mail this application with your check or money order
made payable to ABCM for $250.00. No online payments allowed for pen and paper exams.

To retake your CVRN Level I exam online, click on the add to cart button below;
associated cost is $120.00. Allow up to 3 hours to retake this examination;
the average exam time has been 2 hours. The exam is timed and will automatically end without notice.

   CVRN Level II Board Certification Examination
            (CCU, CVICU, Cardiac Surgery and Special Procedures Labs)

            Prerequisite: do not attempt to take this exam if you have not already passed the CVRN
            Level I Exam above.

To take your exam online, click on the add to cart button; associated cost is $150.00.   Allow up to 2 hours for this
examination; the average exam time has been 1.5 hours. The exam is timed and will automatically end without notice.

To take your exam by pen and paper, mail your check with this application.
Associated cost is $180.00

To retake your exam online, click on the add to cart button below;
associated cost is $75.00. Allow up to 2 hours for this examination; the average exam time has been 1.5 hours.
The exam is timed and will automatically end without notice.

CVRN Level III Board Certification Examination
Cardiac Nurse in Interventional Cardiology Practice; Cath Labs, CV Diagnostic Labs, etc.
Prerequisite: do not apply for this exam unless you have already passed the CVRN Level I Board Certification Exam. Candidates are eligible for this award if they pass the CVRN Level I Exam and already hold a current RCIS certification in order to be endorsed as a Level III CVRN

To be endorsed at level III click on the add to cart button below and pay the $50.00 endorsement fee. Proof of CVRN Level I and a current RCIS certification will be required before endorsement can be made.

CVRN Level IV Board Certification Examination
        (Cardiac Nurses Practitioners, Cardiovascular Nurse Specialists, and Cardiovascular
         Nursing Educators)

         Prerequisite: do not apply for this level unless you have already passed one of the CVRN
         Board Certification exam levels above.

This exam will be available May 15, 2008; associated cost is $150.00. Registration will open online May 15, 2008.
